A personalized hormone therapy combining estriol and progesterone over a twelve-month period significantly reduced menopausal brain fog and improved cognitive function in a small pilot study published in the journal Scientific Reports, according to researchers at UCLA Health. The findings offer a potential therapeutic pathway for cognitive symptoms that currently lack an FDA-approved treatment option. Many women navigating the menopausal transition experience memory lapses, trouble concentrating, and slower thinking speeds, which are commonly described as brain fog. UCLA Health researchers evaluated 20 menopausal women with a mean age of 53 years in an observational case series to see if a tailored hormone regimen could address these complaints. Before starting the intervention, the participants underwent cognitive symptom evaluations.
Understanding Estriol and Brain Protection
Estriol is a natural form of estrogen produced during pregnancy that is already utilized across Europe and Asia to manage hot flashes and other menopausal symptoms, according to Infobae. Unlike estradiol, which is the most common estrogen therapy used in the United States, estriol binds primarily to a different estrogen receptor in the brain. Women are often told that they just have to live with brain fog, when in reality there is a neurobiological basis for it and potentially a way to address it. That care gap is what led us to study estriol more closely.
Preclinical Findings and Study Limitations
To investigate the biological mechanisms behind the clinical observations, the research team administered estriol to middle-aged female laboratory mice. Despite the positive outcomes reported by the human participants and the supporting animal experiments, the authors emphasize that the human study was a small, preliminary case series lacking a control group or randomization. Because of this design, the findings cannot yet be generalized to wider populations. Larger, placebo-controlled clinical trials incorporating standardized cognitive testing and advanced brain imaging will be required to confirm the efficacy of the approach, according to study disclosures. Furthermore, estriol is not approved by the U.S. Food and Drug Administration and remains available domestically solely as a compounded medication, as noted by Infobae.
